Job summary

Harbour At Home is seeking a Care Partner to support aged care participants under the Support At Home program in Woolgoolga and surrounds. You will build trusted relationships with clients and families, guiding care journeys and making budget-minded decisions to improve daily living.

You will collaborate with experienced colleagues and use digital tools to stay organized, delivering high‑quality, goal‑driven care in a supportive team environment.

Qualifications

• Demonstrated empathy and strong communication skills with clients and families.
• Ability to plan, review and adjust care plans in collaboration with participants.
• Experience managing budgets, funding and consent under home care programs.

Responsibilities

• Translate participant goals into personalised, compliant care plans.
• Regular reviews and proactive updates as needs change.
• Plan and track participant budgets to support goals and value for money.
• Guide informed decisions about services, priorities and co-contributions.
• Identify risks and act promptly to maintain wellbeing and service gaps.
• Coordinate services and referrals for seamless, reliable care.
• Maintain accurate documentation and respond to incidents professionally.
• Collaborate with care workers, clinicians and internal teams to ensure quality care.
