Harbour At Home in Woolgoolga is seeking a passionate Care Partner to support aged care participants under the Support At Home program. You’ll build trusted relationships, guide and advocate for participants, and work with families to ensure care plans optimise outcomes while managing budgets.

You’ll be supported by experienced colleagues, with clear processes and digital tools, and you'll focus on meaningful goals, proactive planning, and high-quality, person-centred care.

Qualifications

• Candidates should demonstrate empathy, resilience and strong people skills.
• Experience with older adults and home care is preferred.
• Familiar with budget discussions and funding considerations.

Responsibilities

• Translate participant goals into clear, practical care plans that are personalised and compliant.
• Keep care plans current through regular reviews and updates as needs change.
• Actively plan and track participant budgets to support goals and value for money.
• Guide participants through informed decision-making around services and funding.
• Identify risks and care gaps early and take timely action.
• Coordinate services and referrals to ensure seamless, reliable care.
• Maintain accurate, audit-ready documentation and records.
• Manage incidents, feedback and complaints with professionalism.
