Job summary

Anglicare Sydney is seeking a Care Partner to support older Australians in our Warriewood Brook Village. You will guide residents through their care journey, access My Aged Care funding, and coordinate services to enable ageing in place with dignity.

You will work with village teams and healthcare professionals to assess needs, develop plans and ensure ongoing support, while delivering compassionate, customer-focused service in a dynamic not-for-profit environment.

Qualifications

  • Diploma in Community Services or related discipline required.
  • Experience in aged care, community services or case management.
  • Current Driver's License and access to a vehicle.

Responsibilities

  • Plan and coordinate resident care and case management.
  • Deliver excellent resident experience and customer service.
  • Maintain accurate care plans, notes and documentation.
  • Coordinate referrals to healthcare professionals and community supports.
  • Engage with residents, families and external partners to achieve outcomes.
