Job summary

Melbar is a leading Aged Care Service provider delivering in-home care across Sydney metropolitan areas. We are seeking a full-time Care Partner to assess, coordinate and manage comprehensive home care services for clients.

You will work within a supportive team, focusing on holistic clinical and nursing care, ensuring safe and effective delivery aligned with client health outcomes.

Qualifications

• Minimum Certificate III in aged care, community services or health services.
• Mandarin language proficiency is essential.
• Valid Australian driver’s license with access to a vehicle.
• Experience in community/case management or nursing roles.
• Experience with Aged Care Quality Standards.
• Know Support At Home program.

Responsibilities

• Conduct care assessments and develop person-centred care plans.

JOB DESCRIPTION

Job Description

• Conduct care assessments, develop person-centred care plans and individualised budgets based on participants' needs, goals and preferences.
• Coordinate and manage services with Service Coordinators, Registered Nurses, allied health professionals and other relevant providers.
• Monitor and review participants' wellbeing, care needs, service delivery and outcomes, and update care plans as required.
• Monitor individualised budgets, service utilisation and expenditure to ensure appropriate use of available funding.
• Identify, assess and manage risks, incidents, complaints and safeguarding concerns, and elevate clinical issues to relevant professionals.
• Maintain accurate participant records, provide ongoing support and education, and ensure compliance with aged care legislation, quality standards and organisational policies.

Our Care Partners play a vital role in assessing, coordinating, and managing the care services for our support at Home clients. As part of a vibrant and dynamic team, the Care Partners ensure our clients receive a holistic approach to clinical and nursing care, with focus on the needs of clients, they provide comprehensive, safe and effective evidence-based care to achieve our clients' individual health outcomes.

About the Candidate

• A minimum Certificate III in Individual Support (Ageing), Certificate III in Health Services Assistance, or another relevant Certificate III qualification in aged care, community services or health services.
• Bilingual skills are essential with Mandarin as a compulsory language;
• Valid Australian driver's license with access to own vehicle,
• Demonstrated experience in a community/ case manager / nursing role;
• Experience in care management and the Aged Care Quality Standards
• Know Support At Home program
