Job summary

Responsibilities

• Provide end-to-end support for home care packages, including assessment and commencement
• Identify client strengths and deliver appropriate care
• Develop, review, and update care plans to support goals
• Liaise with medical, allied health, and community stakeholders
• Maintain monthly client contact and monitoring
• Implement and manage client budgets
• Ensure compliance with Aged Care Quality Standards

Workforce Extensions Sunshine Health is seeking a compassionate and reliable Care Partner for our client, Solara Health. If you have experience supporting older people in their homes and are passionate about providing high-quality, person-centered support, we’d love to hear from you.

What do you need?

• Qualifications or experience in Nursing is preferred
• Experience in Case Management is preferred
• Strong time management and organisational skills
• Excellent verbal and written communication
• Ability to build positive relationships with clients, families, and advocates
• Ability to work autonomously and in a team environment
• Current Police Check, driver’s licence

Duties & Resposibilities

• Provide end-to-end support for clients receiving Support at Home Packages, including assessment and commencement
• Work with clients to identify strengths and abilities and deliver appropriate care and services
• Develop, review, and update care plans to support client goals
• Liaise with medical, allied health, and community stakeholders
• Maintain regular client contact, including monthly monitoring calls
• Implement and manage client budgets
• Ensure compliance with Aged Care Quality Standards
• Speaking a second language is an advantage
• Vehicle provided/own: company vehicle provided
• Pay rates: $80K + super
• Reporting to: Community Care Manager
