Job summary

Melbar is seeking an energetic and driven Full-time Care Partner to join a vibrant team delivering in-home aged care services across the Sydney metropolitan area. You will assess, coordinate, and manage care plans, focusing on client needs with an emphasis on quality and safety.

You will work with a supportive service coordination team and be offered flexible hybrid work up to 3 days at home, with opportunities for incentives and career development.

Qualifications

• Relevant qualifications in Care with Community experience e.g. Certificate IV in Aged Care and/or Home and Community Services, Care management or Nursing
• Demonstrated experience in a community/case manager/nursing role
• Coordinating assessments & care planning
• Strong problem-solving and leadership skills
• Experience in care management and the Aged Care Quality Standards
• High level of organisational skills and details orientated
• Excellent verbal and written communication skills
• Bilingual skills are essential with Mandarin as a compulsory language
• Valid Australian driver’s license with access to own vehicle
• Competent computer skills in Microsoft Office software and ability to learn business systems quickly
• Demonstrate familiarity with Support At Home program, having previous experience in a home care environment will be a bonus.

Responsibilities

• Assess, coordinate, and manage the care services for our Support At Home clients
• Provide holistic clinical and nursing care tailored to client needs
• Collaborate with service coordination (rostering), finance and intake teams to enable effective case management
• Deliver safe, evidence-based care and work toward individual health outcomes

The Opportunity:

Our Care Partners play a vital role in assessing, coordinating, and managing the care services for our Support At Home clients. As part of a vibrant and dynamic team, the Care Partners ensure our clients receive a holistic approach to clinical and nursing care, with focus on the needs of clients, they provide comprehensive, safe and effective evidence-based care to achieve our clients individual health outcomes.

As a Care Partner, you will be supported by our service coordination (rostering) team, the finance team to look after reimbursements and Intake team for new clients to enable you to focus on providing quality case and client management service.

Experience and Skills:
