Job summary

Melbar is seeking an energetic and driven Full-time Care Partner to join our supportive team in Sydney. You will assess, coordinate and manage care services for at-home clients, delivering holistic clinical and nursing care with a focus on client needs and outcomes.

Key responsibilities include developing care plans, coordinating with nurses and allied health staff, monitoring client wellbeing and budgets, and ensuring compliance with aged care standards and policies.

Qualifications

• Minimum Certificate III in Ageing, Community Services or Health Services.
• Mandarin language is essential.
• Valid Australian driver’s license with access to own vehicle.
• Demonstrated experience in a community/case manager or nursing role.
• Experience in care management and Aged Care Quality Standards.
• Know Support At Home program.

Responsibilities

• Conduct care assessments and develop person-centred care plans and budgets.
• Coordinate and manage services with Service Coordinators, RNs and allied health professionals.
• Monitor wellbeing, care needs and outcomes; update care plans as required.
• Monitor budgets, service utilisation and expenditure to ensure funding usage.
• Identify, assess and manage risks, incidents, complaints and safeguarding concerns.

Melbar is looking for an energetic and driven Full-time Care Partner to join our supportive and dedicated team.

Our Care Partners play a vital role in assessing, coordinating, and managing the care services for our support at Home clients. As part of a vibrant and dynamic team, the Care Partners ensure our clients receive a holistic approach to clinical and nursing care, with focus on the needs of clients, they provide comprehensive, safe and effective evidence-based care to achieve our clients' individual health outcomes.

Key responsibilities

• Conduct care assessments, develop person-centred care plans and individualised budgets based on participants’ needs, goals and preferences.
• Coordinate and manage services with Service Coordinators, Registered Nurses, allied health professionals and other relevant providers.
• Monitor and review participants’ wellbeing, care needs, service delivery and outcomes, and update care plans as required.
• Monitor individualised budgets, service utilisation and expenditure to ensure appropriate use of available funding.
• Identify, assess and manage risks, incidents, complaints and safeguarding concerns, and elevate clinical issues to relevant professionals.
• Maintain accurate participant records, provide ongoing support and education, and ensure compliance with aged care legislation, quality standards and organisational policies.

Requirements

• A minimum Certificate III in Individual Support (Ageing), Certificate III in Health Services Assistance, or another relevant Certificate III qualification in aged care, community services or health services.
• Bilingual skills are essential with Mandarin as a compulsory language;
• Valid Australian driver’s license with access to own vehicle
• Demonstrated experience in a community/ case manager / nursing role;
• Experience in care management and the Aged Care Quality Standards
• Know Support At Home program
