Job summary

BlueCare is seeking a Care Partner to ensure person-centred, high-quality care across Bowen, Proserpine and surrounding areas, with travel between offices. You will oversee care planning, coordinate services and work with clinical teams to optimise outcomes for community clients.

Responsibilities

• Develop and review care plans in collaboration with the client and multidisciplinary care team.
• Manage budgets and facilitate multidisciplinary care activities.
• Liaise with clinical teams to monitor and respond to changing needs of complex community clients.
• Continuously monitor, review and evaluate care plans, services and outcomes.
