Job summary

BlueCare is seeking a Clinical Care Partner to coordinate complex clinical and care needs in Lowood and surrounding areas. You will assess, plan and link services to aged care funding, working with a multidisciplinary team to improve client outcomes.

The role requires AHPRA registration, a valid driver’s licence and the ability to travel. You’ll join a trusted community and residential care provider, with salary packaging and generous benefits.

Qualifications

• Registered Nurse or Allied Health professional with current AHPRA registration.
• Understanding of client-centred, community-based care delivery.
• Experience with community funding streams and programs.
• Excellent communication and clinical problem-solving skills.
• Current Australian Driver’s Licence and vehicle for work.

Responsibilities

• Develop and review complex care plans with clients and multis disciplinary team.
• Manage budgets and facilitate multidisciplinary care activities.
• Liaise with clinical teams to monitor changing needs of complex community clients.
• Monitor, review and evaluate care plans, services and outcomes.
• Assist clients in navigating the care system and connect to resources.
• Spend most days on the road engaging with clients in the community.
