Job summary

BlueCare Care Partner supports clients in Redland Bay, Victoria Point and surrounding areas, delivering person‑centred care and coordinating services with clinical teams. You will develop and review care plans, manage budgets, and liaise with multidisciplinary teams to achieve the best outcomes for complex community clients.

You will spend significant time on the road delivering care, with autonomous planning and a focus on high-quality service.

Qualifications

• Required Cert IV in Health or Community Service (or equivalent).
• Current Australian Driver’s Licence required.
• Vehicle is required for work and must be roadworthy and insured.

Responsibilities

• Develop and review care plans in collaboration with clients and care teams.
• Manage budgets and coordinate multidisciplinary care activities.
• Liaise with clinical teams to monitor changing needs of complex clients.
• Continuously monitor, review and evaluate care plans and outcomes.
• Assist clients in navigating the care system and linking to resources.

Working closely with the clinical teams to achieve the best care outcomes for each client from their Aged Care Service funding through activities that include:Develop and review care plans in collaboration with the client and multidisciplinary care teamManage budgets and facilitate multidisciplinary care activitiesLiaise with clinical teams to monitor and respond to changing needs of complex community clientsContinuously monitor, review and evaluate care plans, services and outcomesAssist clients in navigating the care system and linking them to necessary resources where requiredWith no two days ever the same.
