Job summary

Fresh Hope Communities is seeking a Care Manager (RN) to join our 166-bed East Maitland home. You will lead clinical care on shift, mentor staff and drive resident wellbeing under our quality framework and accreditation standards.

You are a compassionate, hands-on leader with AHPRA registration and aged care experience, capable of coordinating care across households, collaborating with families and healthcare professionals, and maintaining calm, patient-centred operations in a busy setting.

Due to internal promotion, we're seeking a Care Manager to join the team at our 166-bed home in East Maitland. With our Care Managers working side by side responsible for different households reporting to our Clinical Services Manager and a dedicated Facility Manager, this role allows you to apply your clinical expertise and management skills in a service with a strong reputation for quality care.

Your Day-to-Day

• Lead and inspire a positive culture for your team aligned with Fresh Hope Communities' values,
• Oversee clinical care on shift, contribute to a culture of clinical excellence, and addressing any gaps in care and ensuring timely resolution,
• Foster resident-centred care which meets the physical, psychological, emotional, and spiritual needs of our residents,
• Collaborate with healthcare professionals and families to optimise resident wellbeing and outcomes,
• Ensure compliance with accreditation and quality standards.

A bit about You:

As a Registered Nurse with AHPRA registration, you will have

• Experience in aged care and knowledge of Aged Care Quality Standards,
• Demonstrated clinical and people leadership skills with the ability to coach and mentor staff,
• Compassionate, hands-on leader with a commitment to resident wellbeing and clinical excellence,
• Ability to multitask with shifting priorities and work in a calm considered manner,Have strong verbal and written communication skills,
• A current Criminal History Check or willingness to apply for a new one.
