Job summary

Bolton Clarke is seeking a Care Manager to lead clinical governance, staff management, change management, AN-ACC and resident care. The Seaton Place facility in Cleveland, QLD, is a brand new 128-bed home with strong community links, offering modern facilities and ample parking.

You will ensure high quality, personalised resident care, supervise RNs and PCWs, mentor teams, drive accreditation readiness and continuous improvement, and uphold Bolton Clarke policies.

Qualifications

• Current Nursing Registration with AHPRA.
• Experience in Aged Care as a Senior Registered Nurse or Clinical Nurse Leader (desirable).
• Infection Prevention and Control qualification (desirable).
• Solid knowledge of aged care standards, accreditation, auditing, and AN-ACC.
• Commitment to high quality care, staff development and continuous improvement.
• Leadership and strong communication skills.
• Valid working rights in Australia.
• Must complete a police check or NDIS Worker Check before commencement.

The Role

As the Care Manager, your responsibilities will expand across clinical governance, staff management, change management, AN-ACC and resident care. Your duties will include, but are not limited to:

• Commitment to ensuring the delivery of high quality and personalised care to all resident
• Knowledge of the aged care sector, standards
• Sound understanding of processes needed to respond to clinical and quality audits using the principles of continuous improvement.
• Developing, monitoring and implementing nurse care plans + assessing, planning and evaluating existing care plans
• Performing comprehensive assessments and investigating issues and incidents
• Participating in and ensuring all compliance is adhered to in accordance with aged care standards and Bolton Clarke policies and procedures.
• Actively supervising, training, and mentoring all RNs and PCWs

About You

The successful candidate must meet the following criteria:

• Current Nursing Registration with AHPRA
• Experience within Aged Care Sector as a Senior Registered Nurse or Clinical Nurse Leader (desirable)
• Infection Prevention and Control Qualification (desirable)
• Demonstrated working knowledge of current aged care standards, accreditation and auditing processes, and AN-ACC
• Personal and professional commitment to delivering high quality care, the development of yourself and your staff, and the overall continuous improvement of the home
• Solid leadership skills along with excellent communication and interpersonal skills
• Valid working rights in Australia (Passport, Birth Certificate etc)
• Complete a police check and or a NDIS Workers Check (or willingness to obtain) prior to commencement
