Job summary

Bolton Clarke is seeking a Care Manager to lead clinical governance, staff management and resident care across a Western Sydney site. The role focuses on ensuring high-quality, personalised care, audit response, and continuous quality improvement within our aged care setting.

You will supervise RNs and PCWs, develop and monitor care plans, assess and evaluate resident needs, and ensure compliance with national standards and organisational policies.

Qualifications

• Current Nursing Registration with AHPRA.
• Experience in the Aged Care sector as a Senior Registered Nurse or Clinical Nurse Leader is desirable.
• Infection Prevention and Control qualification is desirable.
• Strong knowledge of aged care standards, accreditation and auditing processes, including AN-ACC.
• Commitment to high quality care and continuous improvement of the home.
• Solid leadership and excellent communication skills.

Responsibilities

• Ensure delivery of high quality and personalised resident care.
• Respond to clinical and quality audits and drive continuous improvement.
• Develop, monitor and implement nurse care plans; assess, plan and evaluate existing care plans.
• Perform comprehensive assessments and investigate issues.
• Ensure compliance with aged care standards and Bolton Clarke policies.
• Supervise, train and mentor RNs and PCWs.

The Role

As the Care Manager, your responsibilities will expand across clinical governance, staff management, change management, AN-ACC and resident care. Your duties will include, but are not limited to:

• Commitment to ensuring the delivery of high quality and personalised care to all resident
• Knowledge of the aged care sector, standards
• Sound understanding of processes needed to respond to clinical and quality audits using the principles of continuous improvement.
• Developing, monitoring and implementing nurse care plans + assessing, planning and evaluating existing care plans
• Performing comprehensive assessments and investigating issues and incidents
• Participating in and ensuring all compliance is adhered to in accordance with aged care standards and Bolton Clarke policies and procedures.
• Actively supervising, training, and mentoring all RNs and PCWs

About You

The successful candidate must meet the following criteria:

• Current Nursing Registration with AHPRA
• Experience within Aged Care Sector asa Senior Registered Nurse or Clinical Nurse Leader (desirable)
• Infection Prevention and Control Qualification (desirable)
• Demonstrated working knowledge of current aged care standards, accreditation and auditing processes, and AN-ACC
• Personal and professional commitment to delivering high quality care, the development of yourself and your staff, and the overall continuous improvement of the home
• Solid leadership skills along with excellent communication and interpersonal skills
• Valid working rights in Australia (Passport, Birth Certificate etc)
• Complete a police check and or a NDIS Workers Check (or willingness to obtain) prior to commencement
