Job summary

Croydon Health Services NHS Trust is recruiting a Care Home Pathway Coordinator to join the Transfer of Care Hub (TOCH) and manage all aspects of supported discharges and the single point of discharge from CUH to care homes.

The post holder will be based at the Croydon University Hospital NHS Foundation Trust and will coordinate the activities related to the Red Bag journey for patients coming into the Hospital from care homes and ensure the Red Bags are received in the hospital and returned to

Qualifications

• The Care Home Pathway Coordinator collaborates with ward teams and discharge coordinators to plan care home discharges.
• Co-ordinates Hospital Transfer Pathway and ensures discharge documents are finalised prior to discharge.
• Promotes awareness of the Hospital Transfer Pathway and maintains effective communication with partner agencies.

Responsibilities

• Work closely with ward teams and Discharge Co-ordinators to identify care home residents needing discharge planning and keep plans updated.
• Collaborate with discharge teams to manage the Hospital Transfer pathway from admission to discharge.
