Job summary

Croydon Health Services NHS Trust is seeking a Discharge Coordinator to lead complex discharge planning, coordinate the Discharge Navigators and work with ward teams to ensure timely, safe patient transitions to community care. The role requires critical thinking, clear communication, and a proactive approach to identify and resolve barriers to discharge.

The post is full-time across 7 days with flexible scheduling.

Responsibilities

• Provide compassionate care within scope and deliver therapy to agreed quality standards.
• Maintain momentum of discharge planning across the board, coordinating with MDT and Community partners.
• Undertake baseline assessments including Mental Capacity and Care Act assessments with the MDT.
• Represent the Transfer Of Care Hub Team at ward daily board rounds.
• Communicate discharge actions clearly using standard tools and updates.
• Undertake delegated duties with minimal supervision and report back to ward team and case manager.
• Collaborate across multi-professional disciplines and refer for additional assessments/treatments as required.
• Promote the discharge lounge and advocate its use.
• Exercise supervisory responsibility for Discharge Navigators.
