Qualifications

• Works within a multidisciplinary approach to facilitate safe patient discharges.

Responsibilities

• Facilitate safe and effective discharges from hospital to community settings.
• Coordinate patient assessments and support discharge planning.
• Identify potential delays and work with Integrated Discharge Team to overcome them.
• Ensure appropriate referrals to local authorities to prevent discharge delays.
• Collaborate with ward teams, TOCH, MDT and community partners.
• Promote the use of the discharge lounge.
