Job summary

H1 Healthcare in Sydney is seeking an experienced Care Coordinator to join our Sydney team. You will coordinate disability and in-home support for clients across NSW, with a focus on Icare-funded participants.

You’ll juggle priorities, liaise with families and case managers, and ensure clients receive care at home and in the community. Ideal candidates bring strong Icare program experience, disability coordination skills, and the ability to manage rostering and reporting.

Qualifications

• Demonstrated experience working with Icare programs (essential).
• Experience in disability, attendant care or in-home support coordination.
• Solid understanding of Icare service request processes and reporting requirements.
• Exceptional organisational, coordination and rostering skills.
• Outstanding communication and relationship-building abilities.
• Proactive, solutions-focused mindset with the ability to make decisions.
• Ability to manage competing priorities with attention to detail.
• Willingness to travel within NSW to visit clients.
• Current driver’s licence and access to a reliable vehicle.
• Current NDIS Worker Screening Check and Working With Children Check (or willingness to obtain).
• Experience in disability or worksafe sectors and complex/high-needs care is highly regarded.

Responsibilities

• Coordinate personalised, person-centred support plans enabling clients to live independently at home.
• Manage complex care arrangements under Icare schemes (Lifetime Care, Workers Care) per guidelines.
• Visit clients across Sydney/NSW for reviews and assessments.
• Collaborate with families, support workers, case managers and allied health professionals to ensure seamless care.
• Roster and match support workers; respond quickly to changing client needs.
• Prepare service requests, progress reports and documentation to Icare standards.
• Advocate for clients to access funding, services and community resources.
• Monitor service quality and client safety; respond to incidents and feedback.
• Problem-solve and adapt quickly to daily challenges.
