Job summary

Cohort Staffing is seeking an In-Home Complex Care Coordinator in Geelong to lead high-quality, person-centred supports for participants with complex care needs.

You will coordinate in-home care, work with families and health professionals, and ensure care plans and documentation are maintained to a high standard. The role requires 3+ years of complex care experience, with nursing qualification preferred and strong NDIS knowledge.

Qualifications

• Minimum 3 years' experience in complex care, disability, community health, aged care or similar care environment.
• Nursing qualification and current professional registration highly preferred for complex health care roles.
• Strong understanding of NDIS and relevant quality and safeguarding requirements.

Responsibilities

• Coordinate in-home complex care supports, ensuring safety and alignment with participant needs.
• Collaborate with participants, families, support workers, nurses and allied health professionals.
• Develop and maintain strong participant relationships and ensure care plans are followed.
• Monitor participant health, wellbeing, risks and changes in support needs.
• Provide guidance and support to care teams working with participants with complex needs.
• Maintain accurate participant documentation, progress notes, care plans and records.
• Identify and respond to risks, incidents and changes in participant circumstances.
• Work with senior management to identify service improvements and manage emerging issues.
• Ensure compliance with NDIS Practice Standards and organisational policies.

We are looking for an experienced In-Home Complex Care Coordinator to join our team and take a lead role in coordinating high-quality, person-centred supports for participants with complex care needs. This is a hands-on coordination role suited to someone with strong experience in complex disability support, NDIS requirements and the coordination of safe, high-quality care.

What You'll Do

• Coordinate and oversee in-home complex care supports, ensuring services are safe, consistent and aligned with participant needs
• Work closely with participants, families, support workers, nurses, allied health professionals and other stakeholders
• Develop and maintain strong participant relationships and ensure support plans are followed
• Monitor participant health, wellbeing, risks and changes in support needs
• Provide guidance and support to care teams working with participants with complex needs
• Ensure participant documentation, progress notes, care plans and other records are accurate and maintained
• Identify and respond to risks, incidents and changes in participant circumstances
• Work with senior management to identify service improvements and manage emerging issues
• Ensure compliance with NDIS Practice Standards, organisational policies and relevant legislation

What You'll Bring

• Minimum 3 years' experience in complex care, disability, community health, aged care or a similar care environment
• A nursing qualification and current professional registration is highly preferred, particularly for candidates with experience in complex health care, clinical risk management or high-needs disability support
• Strong experience coordinating supports for people with complex health or disability needs
• Strong understanding of the NDIS and relevant quality and safeguarding requirements
• Excellent communication, documentation and stakeholder management skills
• Ability to work independently, prioritise competing demands and make sound decisions
• A practical, participant-focused approach and strong attention to detail

Requirements & Checks

• Minimum 3 years' relevant experience

Minimum Certificate IV qualification in Disability, Community Services or a related field. A nursing qualification with current professional registration is highly preferred.
