Job summary

Hope and Care Community Services in Morayfield, QLD, is seeking a Care Coordinator to oversee service delivery across SIL homes in Brisbane’s Northside, ensuring safety, high-quality supports, and alignment with NDIS Practice Standards.

You’ll lead and support staff, coordinate participant intakes and transitions, manage documentation and incidents, and build strong relationships with participants and families to deliver person-centred supports.

Hope & Care Community Services is an established, grassroots organisation dedicated to supporting people with disabilities to live independently and participate fully in their communities.

As we continue to expand, we are seeking a Care Coordinator to oversee service delivery across Supported Independent Living (SIL) homes across Brisbane’s Northside, ensuring services are safe, well-coordinated, and aligned with NDIS Practice Standards.

Key responsibilities include:

• Service Delivery Coordination: Coordinate service delivery across SIL homes to ensure consistent, high-quality supports.
• Participant Intake & Transitions: Support Participant intakes, transitions, admissions, and exits.
• Site Oversight & Compliance: Conduct site visits to ensure compliance, safety, and service quality standards are maintained.
• Staff Leadership & Support: Lead and support staff through supervision, training, and performance management.
• Documentation & Incident Management: Maintain compliance incidents reporting, documentation, and Participant records.
• Stakeholder Engagement: Build and maintain strong relationships with Participants, families, and key stakeholders.
