Five Good Friends in Brisbane, Australia, is seeking an experienced Care Coordinator to lead assessment, planning and delivery of in-home care for Members. You’ll coordinate care with Helpers and trusted partners, updating plans and ensuring seamless service delivery.
You’ll work closely with Care Specialists, Community Nurses and Workforce Coordinators to tailor solutions that keep Members safe, comfortable and supported at home, with a focus on empathy and practical outcomes.
