The Role

We are seeking an experienced Care Partner, you’ll be the link between clients, families, and support workers. Ensuring care is delivered with empathy, accuracy, and heart. This is a dynamic role where you’ll combine client engagement, care coordination, and rostering responsibilities, making every day varied and rewarding.

Key Responsibilities

• Support clients in navigating their Support at Home funding.
• Build strong, trusting relationships with clients, families, and support staff.
• Coordinate services to match each client’s needs, goals, and preferences.
• Create and manage effective rosters for our team of support workers.
• Respond promptly and empathetically to client enquiries and changing circumstances.
• Collaborate with internal teams and external providers for seamless service delivery.
• Maintain accurate documentation and uphold compliance with sector standards.

About You

• Experience in care coordination, rostering, or client services.
• Knowledge of the NDIS and/or Aged Care sector (especially self-managed NDIS & HCP).
• Excellent communication and time management skills.
• A proactive, can-do attitude with a passion for client care.
• Confident computer skills (rostering systems or CRMs a bonus).
• Police Check, WWCC, NDIS Worker Screening Check (or willingness to obtain).
• Current First Aid & CPR (or willingness to obtain).
• Minimum of two doses of COVID-19 vaccination.
• Comfortable in fast-paced, regulated, or changing environments
• Proactive and committed to continuous improvement
