About the role

Working closely with our Aged Care Services Manager, RN Clinical Governance Lead, Team Leaders and associated providers, you will:

  • Develop, coordinate and review individual care plans with clients and their chosen supporters.
  • Prepare and monitor client budgets, identifying underspending or overspending and working with clients to make informed decisions.
  • Maintain regular contact with clients and respond to changes in their needs, preferences and circumstances.
  • Coordinate services and referrals, liaising with support workers, allied health professionals and associated providers.
  • Assist scheduling staff to address service needs and maintain continuity of support.
  • Respond to prospective client enquiries about services, pricing and commencement.
  • Recognise and promptly escape clinical concerns, incidents and safeguarding issues.
  • Maintain accurate and timely client records in our Customer Management System.
  • Contribute to quality improvement and support recruitment and onboarding activities when required.
About you

  • Experience developing care plans and managing individual client budgets.
  • An understanding of Support at Home, aged care reforms, Quality Standards and client rights.
  • Strong organisational skills and the ability to manage competing priorities.
  • Confidence working with clients, families, carers and service providers from diverse backgrounds.
  • Good written communication and computer skills, including experience with client management systems.
  • A current driver licence and willingness to travel for client visits.

A relevant qualification in aged care, community services, case management or a related discipline is desirable.
