Job summary

Trilogy Care is seeking a dedicated Care Coordinator to join our Brisbane team. You will act as the primary contact for a portfolio of care recipients, coordinating and scheduling services, and maintaining accurate client records and case notes.

You’ll use multiple technology platforms to deliver services, monitor budgets, and source information to support client goals, with a focus on clear communication and collaborative teamwork in a fast-growing platform transforming care in Australia.

We Care Coordination partners with Trilogy Care, a leading self-managed Support at Home provider, to help older Australians live independently at home for longer.

We coordinate services, manage care plans and budgets, and support clients to get the most from their aged care packages with transparency, choice, and genuine value. We’re a growing, forward-thinking team that values people, innovation, and meaningful work.

What You’ll Do
