Job summary

Nepean Blue Mountains Local Health District is seeking an experienced Registered Nurse to deliver in-home care across Western Sydney, enabling older Australians to live safely and independently at home. The role is highly autonomous, working with Care Partners and a broader team to provide clinical support, wound care and ongoing assessments.

You will manage client needs, adapt to changing schedules, document thoroughly, and contribute to quality and compliant service delivery while building

Qualifications

• Experience in aged care, home care or community nursing.
• Strong clinical judgement and ability to work autonomously.
• Comfortable managing own day while staying connected to a team.

Responsibilities

• Deliver nursing care, personal care and clinical support to home care clients.
• Conduct health assessments, reviews and ongoing monitoring.
• Provide wound care and other nursing interventions.
• Identify changes in client wellbeing and recommend supports.
• Collaborate with families and allied health professionals for outcomes.
• Maintain accurate clinical documentation and records.
• Support quality, compliance and safe service delivery.
• Help clients stay independent and living well at home.
