Job summary

Gladstone Community Linking Agency Ltd (GCLA) is seeking an Aged Care Care Partner (SAH) to join our team in Gladstone, QLD. You will coordinate services, develop care plans and monitor wellbeing for participants living independently at home.

The role involves working with participants, families and providers to ensure funding compliance and quality standards are met, with a focus on person-centred care and safeguarding.

Qualifications

• Experience in aged care, care coordination or case management.
• Excellent communication and relationship-building skills.
• Strong organisational and time management skills.
• Ability to work independently and collaboratively.
• Current Australian Driver Licence.
• Current First Aid and CPR Certificate.
• Working with Children (Blue Card)
• NDIS Worker Screening Check (Yellow Card)
• Certificate IV in Ageing Support, Community Services or a related qualification (or equivalent experience).
• Experience working within the Commonwealth Home Support Program (SAH), Home Care Packages or Support at Home.
• Knowledge of the Aged Care Quality Standards and My Aged Care.
• Experience using client management systems.

Responsibilities

• Complete participant assessments, care planning and regular reviews.
• Coordinate services that support participants to remain living safely at home.
• Work closely with participants, families, carers and service providers.
• Monitor participant wellbeing and respond to changing support needs.
• Maintain participant budgets and ensure services align with available funding.
• Maintain accurate documentation and participant records.
• Promote participant choice, independence and dignity.
• Identify and elevate safeguarding, compliance and service delivery concerns.
• Contribute to continuous improvement and quality outcomes.
