Job summary

Central Highlands Rural Health in Victoria seeks a caring Care Partner to join our Home Support Program. You will work with older people and their families to coordinate services, support goal achievement and deliver a person-centred care experience that enables clients to live safely and independently at home.

The role is permanent part-time, with hours negotiated. Salary is HS5 at 48.87 AUD per hour plus superannuation.

Qualifications

• Experience in community/home care is preferred.
• Commitment to person-centred care and compassionate service.

Responsibilities

• Coordinate and monitor services for older people living at home.
• Complete assessments, care planning and reviews.
• Build relationships with clients, families and service providers.
• Support clients to achieve goals and maintain independence.
• Work collaboratively to deliver high-quality care and outcomes.
