Care Connect Group in Sydney is seeking a dedicated Care Manager to join our aged care team on a 12-month maximum-term full-time contract, supporting a caseload across the Greater Sydney region.

You will assess client needs, develop person-centred care plans and connect clients with services to maintain independence at home, while building trusted relationships with clients, families and local service providers.

Formación

• Diploma qualification in health, social or community services or working toward one.
• Excellent communication and relationship-building skills.
• Strong planning and time management abilities.
• Client-centered approach to support older Australians.
• Accurate record-keeping and attention to detail.

Responsabilidades

• Complete client assessments and develop person-centred care plans.
• Support clients to access services promoting independence and wellbeing.
• Monitor care plan and budget reviews.
• Build relationships with clients, families and service providers.
• Collaborate with Service Coordinators to deliver services safely.
• Maintain accurate client records and documentation.
• Provide advice to help clients make informed care decisions.
• Identify opportunities to improve client outcomes.
