Job summary

South Eastern Community Connect in Sydney is seeking an experienced Care Partner to support aged clients in the community from Eastlakes/Mascot. The role is hybrid, full-time or contract, with sponsorship available for the right candidate.

You will work as part of a warm team, providing end-to-end case management, intake, care plan updates and budget monitoring, visiting clients at home as needed.

Qualifications

• Minimum of two years experience in Aged Care.
• Experience as a case manager in Aged Care is preferred.
• Own car and a valid driving license with insurance.
• Strong client-focused case management skills.
• Knowledge of Aged Care Quality Standards and regulatory requirements.
• Excellent communication and organisational abilities.

Responsibilities

• Coordinate end-to-end case management for home care clients.
• Manage intake, care plans, budgets and referrals.
• Visit clients at home as required.
• Ensure services remain within allocated funding.
• Maintain accurate logs of client interactions and updates.
