Care Connect is hiring a Senior Care Manager in NSW to lead a team of Care Managers, ensuring safe, high-quality, person-centred care. You will translate aged care reform and organisational priorities into day-to-day practice, supporting governance and continuous improvement across services.

The role requires leadership experience, strong communication, and a commitment to exceptional client outcomes within a hybrid work setting.

As we continue to evolve our Aged Care services and respond to significant sector reform, we're seeking an experienced and passionate Senior Care Manager to join our growing Aged Care team.

About The Role

Reporting to the Aged Care Lead, the Senior Care Manager provides day-to-day leadership, coaching and practice guidance to a team of Care Managers, ensuring clients receive safe, high-quality, person-centred care that supports their independence and wellbeing.

You’ll play a key role in supporting complex care management decisions, strengthening care governance and quality outcomes, building team capability, and driving service excellence.

As a visible and influential leader, you’ll help translate aged care reform, organisational priorities and quality standards into consistent day-to-day practice.

Key Responsibilities

• Lead, coach and develop a team of approximately 8-12 Care Managers.
• Provide guidance and support on complex client situations and escalations.
• Promote person-centred care, wellness and reablement principles.
• Strengthen quality, safeguarding, care governance and risk management practices.
• Support service performance, client experience and continuous improvement initiatives.
• Build team capability through coaching, mentoring and development.
• Partner with internal teams, providers, clients and families to achieve positive outcomes.
• Support the implementation of aged care reforms and new ways of working.
• Use data, feedback and insights to drive service improvements and operational effectiveness.

About You

You are a collaborative and compassionate leader who enjoys supporting others to succeed and is committed to delivering exceptional client outcomes.

You bring strong operational judgement, excellent communication skills and a passion for leading teams in a complex and evolving aged care environment.

To be successful, you will have:

• Experience in care management, case coordination, team leadership or service management.
• Strong understanding of person-centred care, wellness and reablement principles.
• Experience coaching, mentoring and developing employees.
• Knowledge of aged care quality standards, safeguarding, care governance and risk management.
• Experience within aged care, home care, health, disability or community services.
• Tertiary qualifications in Nursing, Allied Health, Social Work, Community Services or a related field.
• Leadership, coaching or case management qualifications.
• Excellent communication, stakeholder engagement and problem-solving skills.
• Strong organisational skills and the ability to manage competing priorities.
• A commitment to continuous improvement and service excellence.
