Job summary

Care Connect is seeking a Care Manager to join our Aged Care team on a 12-month maximum-term contract (full-time), supporting a caseload of clients across the Greater Sydney region. You will assess needs, develop person-centred care plans and connect clients with services to help them live independently at home.

You will build trusted relationships with clients and families, monitor progress and collaborate with Service Coordinators to ensure safe, responsive care.

Qualifications

• Diploma qualification in health, social or community services.
• Strong communication and relationship-building skills.
• Excellent planning, organisational and time management skills.
• Client-focused approach and genuine desire to support older Australians.
• Strong attention to detail and accurate record-keeping.
• Ability to manage competing priorities and work autonomously.
• Confidence using Microsoft Office and other business systems.

Responsibilities

• Complete client assessments and develop person-centred care plans.
• Support clients to access services that promote independence, choice and wellbeing.
• Monitor client needs and conduct ongoing care plan and budget reviews.
• Build strong relationships with clients, families, service providers and referral partners.
• Collaborate with Service Coordinators to ensure services are delivered safely and effectively.
• Maintain accurate client records and documentation.
• Provide advice and support that helps clients make informed decisions about their care.
• Identify opportunities to improve client outcomes and service delivery.
