Job summary

Care Connect is seeking a Senior Care Manager to lead a team of 8–12 Care Managers, guiding complex client decisions and driving quality outcomes in a hybrid work environment. You will strengthen governance and continuous improvement while translating aged care reform into practical day-to-day practice.

The role requires extensive care management and leadership experience, with a passion for person-centred care, safeguarding and service excellence.

Qualifications

• Experience in care management, case coordination, team leadership or service management.
• Strong understanding of person-centred care, wellness and reablement principles.
• Experience coaching, mentoring and developing employees.
• Knowledge of aged care quality standards, safeguarding, care governance and risk management.

Responsibilities

• Lead, coach and develop a team of approximately 8-12 Care Managers.
• Provide guidance and support on complex client situations and escalations.
• Promote person-centred care, wellness and reablement principles.
• Strengthen quality, safeguarding, care governance and risk management practices.
• Support service performance, client experience and continuous improvement initiatives.
• Build team capability through coaching, mentoring and development.
• Partner with internal teams, providers, clients and families to achieve positive outcomes.
• Support the implementation of aged care reforms and new ways of working.
• Use data, feedback and insights to drive service improvements and operational effectiveness.
