CO.AS.IT. Italian Assistance Association in Carlton, Melbourne, seeks a Care Partner to plan and coordinate elder care for participants, liaising with families and the clinical team to deliver culturally responsive services.

You will manage a caseload, set up budgets, and monitor funding while supporting participants to maintain independence across Melbourne’s Northern, Eastern and Western suburbs.

Formación

• Qualification in aged care, community services or health discipline, or equivalent experience.
• Experience supporting older people in home and community settings.
• Strong coordination, assessment and care planning skills.
• Experience monitoring participant budgets and service expenditure.
• Knowledge of the Support at Home program and aged care quality standards.

Responsabilidades

• Assess participants' needs and develop personalised care plans together with them and their families.
• Coordinate services and act as a trusted point of contact for participants and their carers.
• Review care plans regularly and respond to changing needs.
• Set up and monitor participant budgets, ensuring services remain within available funding.
• Work with Support Workers, clinical staff and service providers to support positive outcomes.
• Maintain accurate records in Lookout and My Aged Care.
• Respond to concerns, report incidents and contribute to service improvements.
