Job summary

St Vincent’s Health Australia in Melbourne is seeking a Clinical Assessor to deliver holistic assessments of frail older people in community and hospital settings. You will involve clients and carers in planning and coordinate referrals to support positive outcomes.

The role covers both clinical and non-clinical assessments, home risk screening, and My Aged Care triage, contributing to long- and short-term care plans within a multidisciplinary team.

Qualifications

• Current AHPRA Registration (if applicable)
• Eligibility for membership of Allied Health Professional Association
• Current Victorian Driver’s Licence
• Nationally Coordinated Criminal History Check or willingness to obtain

Responsibilities

• Deliver timely, holistic assessments of frail older people in community and hospital settings.
• Involve clients and carers in developing tailored support plans and coordinate referrals.
• Conduct holistic clinical (and non-clinical where required) assessments of older people.
• Review support plans, undertake home risk screening and complete My Aged Care triage.
• Develop short- and long-term plans addressing health, physical, social and functional needs.
• Participate in team case conferences and implement recommendations to improve care.
• Coordinate with clients, carers, referrers and service providers to optimise outcomes.
