Job summary

APM is seeking a Clinical Aged Care Assessor to evaluate needs of older Australians and connect them with services to support independent living. Daily travel within the service area is required to meet client needs.

You will conduct thorough assessments, refer to essential services, promote functional independence, and collaborate with families and carers while ensuring compliance with health contract requirements. Training and professional development are provided.

Qualifications

• 1 year working with older Australians, including client assessments and/or service delivery.
• Tertiary education in a health-related discipline with professional registration (e.g. AHPRA).
• Proficiency in Word, Excel, Outlook and client management systems.

Responsibilities

• Conduct thorough Aged Care Assessments to identify needs of older adults.
• Facilitate referrals to essential services supporting independent living.
• Promote retention and maximisation of function in daily living.
• Engage with families and carers as key stakeholders.
• Ensure assessments align with Department of Health contractual requirements.
