APM in Bunbury, WA is seeking a part-time Clinical Aged Care Assessor to identify the needs of older adults and connect them with services that promote independence.

This role requires daily travel within the service area to meet client needs, conduct high-quality assessments, and arrange referrals to essential supports. Join a multidisciplinary team focused on improving quality of life for older Australians through compliant, person-centred care.

Qualifications

• At least 1 year working with older Australians, including client assessments and/or service delivery.
• Tertiary education in a health-related discipline together with professional registration with relevant association (e.g. AHPRA).
• Proficiency in Word, Excel, Outlook, and client management systems.
• Current Australian driver’s licence, comprehensively insured vehicle, and the ability to travel within your service area.
