Job summary

Aspire4Life is seeking a Clinical Aged Care Assessor to join our Gippsland team across Traralgon, Bairnsdale and surrounding suburbs. You will conduct holistic home-based aged care assessments helping older Australians access services and maintain independence.

Ideal candidates are qualified clinicians with aged care assessment experience, strong clinical reasoning, and excellent communication. Flexible, Not-for-Profit salary packaging and a generous sign-on bonus are available.

Qualifications

• Experience in aged care assessments.
• Knowledge of MAC frameworks and processes.
• Strong communication and stakeholder engagement skills.

Responsibilities

• Perform holistic, person-centred aged care assessments.
• Develop and monitor support plans focused on wellness and independence.
• Identify supports, services, and community connections for clients.
• Utilise MAC and Integrated Assessment Tool in assessments.
• Collaborate with clients, families, and healthcare providers to coordinate care.
• Manage workload to deliver high-quality outcomes.
