About the role:

We are seeking a dedicated and passionate Activities Worker to join our team on a Part-time basis working Monday and Thursday (08:30am to 15:30pm) each week.

The Activities Worker will assist clients to participate in social support programs that are reflective of the goals and priorities of the clients, meeting their care needs. Social support programs aim to allow our clients to remain engaged with their communities through a range of service options that are delivered in line with client preferences.

Social support programs are offered through Centre based individual and group programs that are respectful and reflective of the goals and priorities of the clients, as well as through the use of virtual platforms and 1:1 home and community support.

The Activities Worker will liaise with the Centre Coordinator, Team Leader and/or Operations Manager to determine the strengths, skills, interests and care needs of clients and ensure all clients have an up-to-date assessment and service plan to support their participation in the program.

In this role, you will:
