Job summary

Uniting AgeWell in Geelong is seeking Personal Care Workers to deliver high-quality, person-centered care to residents within a multidisciplinary team. You will provide compassionate personal care, support residents with complex needs, and assist with care planning, documentation and outcomes evaluation.

Ideal candidates have Certificate III in Aged Care, experience in residential or home care, and a commitment to our BEST values.

Qualifications

• Certificate III in Individual Support, or equivalent (Aged Care).
• Experience in residential aged care/ home care.
• Experience with dementia care and palliative care support (desired).
• Demonstrated abilities to support residents in a fast-paced environment with exemplary communication skills in a team and one-on-one interactions with all levels of care partners

Responsibilities

• Deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs
• Provide personal care to residents with complex needs such as palliative care, dementia support, restorative care and pain management to enhance their wellbeing
• Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes
• Monitor the cleanliness, safety, environment and promptly report issues of concern

About the role:

At Uniting AgeWell, Personal Care Workers co-ordinate personal care for residents in accordance with care plans which meet their individual needs and preferences. Working within a multidisciplinary team, provide effective clinical and emotional support to residents, recognise changes in residents’ conditions and elevate observations and concerns.

• Deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs


• Provide personal care to residents with complex needs such palliative care, dementia support, restorative care and pain management to enhance their wellbeing


• Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes


• Monitor the cleanliness, safety, environment and promptly report issues of concern


• Week 1, Thursday, Friday 07:00 - 15:00, Week 2, Thursday, Friday, Saturday, 07:00 - 22:00


• Thursday, Friday, Saturday, 16:30 - 22:00. Available from the 28th of September until the 29th of March

About you:
