Qualifications

• Bachelor's degree in any discipline.
• Fluent in Arabic and English (written and spoken).
• Minimum 2 years of experience in maternity services, women's health, patient navigation, care coordination or related fields.
• Experience coordinating high-risk obstetric patients and multidisciplinary care coordination preferred.

Responsibilities

• Coordinate patient journeys across antenatal, intrapartum, postnatal and discharge phases.
• Facilitate maternity registration, onboarding, scheduling investigations and referrals.
• Coordinate multidisciplinary reviews and care planning for high-risk pregnancies.
• Support patient education and navigation of hospital services and resources.
• Maintain accurate documentation and monitor maternity pathway KPIs and patient experience.

The Maternity Care Coordinator provides comprehensive care coordination and administrative support across all stages of the maternity pathway The role is particularly focused on ensuring exceptional support for Routine antenatal patientsHigh-Risk Pregnancy HRP patientsPeople of Determination POD The successful candidate will help coordinate appointments referrals investigations and multidisciplinary services while ensuring compliance with Department of Health DOH standards and maternity care pathways

Key Responsibilities

Maternity Care Coordination

Coordinate patient journeys across antenatal intrapartum postnatal and discharge phases Facilitate maternity registration and onboarding processes Schedule and coordinate appointments investigations ultrasounds laboratory tests and follow-up visits Monitor patient progress and ensure critical milestones are completed Support patients in navigating hospital services and available resources Maintain ongoing communication regarding appointments care plans and next steps

High-Risk Pregnancy Support

Serve as a dedicated coordinator for high-risk maternity patients Coordinate specialist referrals including Maternal Fetal Medicine services Facilitate multidisciplinary team reviews and care planning Monitor adherence to investigations consultations and treatment recommendations Track follow-up requirements and elevate coordination concerns when necessary

People of Determination POD Support

Coordinate individualized support plans and accessibility requirements Arrange reasonable accommodations and communication support services Ensure patient education and information are available in accessible formats Advocate for patient-centered and inclusive maternity care

Patient Communication amp Experience

Act as a primary point of contact for maternity patients and families Foster culturally sensitive respectful and compassionate communication Facilitate communication between patients and healthcare teams Escalate urgent patient concerns to the appropriate healthcare professionals Support patients understanding of their care journey and available services

Education amp Engagement

Connect patients and families with approved educational resources Promote participation in antenatal postnatal breastfeeding parenting and newborn care programs Support informed decision-making and patient engagement throughout pregnancy and postpartum recovery

Continuity of Care amp Discharge Planning

Coordinate referrals consultations diagnostics and follow-up care Support discharge planning and arrange post-discharge appointments Conduct post-discharge wellness and service follow-up calls Facilitate access to home care lactation support and community-based services

Quality Improvement amp Reporting

Maintain accurate documentation and tracking systems Monitor maternity pathway metrics KPIs and patient experience indicators Assist with audits quality initiatives and regulatory compliance activities Contribute to departmental performance goals and continuous improvement efforts

Qualifications

Bachelor's Degree in any discipline. Fluent in Arabic and English (written and spoken). Additional languages such as Urdu are an advantage. Minimum 2 years of experience in: Maternity services. Women's health. Patient navigation. Care coordination. Case management and Patient experience. Experience supporting high-risk obstetric patients and multidisciplinary care coordination is preferred.
