Placements24 is seeking a dedicated Social Worker to join the Community Health team in Klerksdorp, North West. The hybrid role involves on-site fieldwork and remote administrative tasks to support individuals and families with psychosocial needs.

You will conduct assessments, provide counseling, develop care plans, connect clients with services, and advocate within the health system, contributing to improved community health outcomes in the North West province.

Qualifications

About the Role Our client is looking for a dedicated and experienced Social Worker to join their Community Health team in Klerksdorp, North West . This hybrid role is designed for professionals who are passionate about improving health outcomes and providing crucial support to individuals and families within the community. You will be responsible for conducting assessments, offering psychosocial support, and connecting clients with necessary health and social services. This position requires a blend of on-site fieldwork in Klerksdorp and surrounding areas, and remote administrative tasks, offering a dynamic work environment. The ideal candidate will possess strong clinical skills, a deep understanding of community health challenges, and the ability to work effectively with diverse populations. Key Responsibilities

• Conduct comprehensive psychosocial assessments of individuals, families, and groups.
• Provide counseling and support services to clients dealing with health-related issues, chronic illnesses, mental health concerns, and life transitions.
• Develop and implement individualized care plans in collaboration with clients and healthcare teams.
• Facilitate access to community resources, healthcare services, and social support networks.
• Advocate for clients' rights and needs within the healthcare system and broader community.
• Deliver health education and promotion programs within the community.
• Collaborate with doctors, nurses, therapists, and other professionals to ensure integrated patient care.
• Maintain accurate, confidential client records and prepare reports as required.
• Participate in case conferences and multidisciplinary team meetings.
• Stay current with relevant legislation, policies, and best practices in social work and community health.

Requirements

• A registered qualification as a Social Worker (e.g., BSW or equivalent) with the South African Council for Social Service Professions (SACSSP).
• Minimum of 4 years of post-qualification experience in a social work or community health setting.
• Proven experience in counseling, case management, and advocacy.
• Strong understanding of health systems, public health principles, and community dynamics in South Africa.
• Excellent communication, interpersonal, and problem-solving skills.
• Ability to work effectively in both community settings and remotely, including proficiency with relevant digital tools.
• Valid driver's license and willingness to travel within the North West region.
• Empathy, resilience, and a commitment to social justice.
• Proficiency in at least two South African languages is an advantage.
• Knowledge of local resources in Klerksdorp and surrounding areas.
