Placements24 is seeking a compassionate Social Care Support Worker to join our team in Pretoria. You will work with a team of professionals, advocating for clients' needs and connecting them to community resources while maintaining confidentiality and ethical practice.

Qualifications

• Relevant qualification in social work, community development, or healthcare support.
• Experience with vulnerable populations preferred.

• Strong interpersonal skills and the ability to build rapport with diverse clients.
• Driver's license and access to a reliable vehicle for fieldwork.
• Proficiency in basic computer skills for remote record-keeping and communication.

Responsibilities

• Provide direct support and assistance to clients in homes and communities, based on individualized care plans.
• Assist clients with daily living activities, such as personal care, meal preparation, and medication reminders.
• Facilitate client participation in community activities, appointments, and social events.
• Monitor client well-being and report any changes or concerns to the supervising social worker or case manager.
• Help clients develop and maintain life skills, fostering independence and self-reliance.
• Maintain accurate and confidential records of client interactions and progress, both in the field and remotely.
