Placements24 is seeking an experienced Lead Care Coordinator to oversee and enhance community support services in George, Western Cape. You will lead a team of care professionals, coordinate client care plans, and ensure high-quality service delivery for a diverse client base.

The role emphasizes collaboration with healthcare providers, families, and community resources, along with strong organizational skills and empathy.

Qualifications

• Bachelor's degree in Social Work, Nursing, Health Sciences, or a related field.
• Minimum of 4 years of experience in community care or related social services role.
• At least 1 year of team leadership or supervisory experience.
• Demonstrated knowledge of client assessment, care planning, and case management.
• Strong understanding of local health and social support systems in the Western Cape.
• Exceptional organizational, time management, and multitasking abilities.
• Excellent interpersonal and communication skills, both written and verbal.

Responsibilities

• Oversee the daily operations of the care coordination team.
• Develop, implement, and monitor client care plans.
• Manage and allocate caseloads among care coordinators.
• Provide guidance, training, and performance feedback to the care team.
• Liaise with healthcare providers, families, and community resources.
• Ensure adherence to regulatory requirements and organizational policies.
• Facilitate interdisciplinary team meetings to discuss client progress.
• Handle client escalations and resolve complex care challenges.
• Contribute to service improvement initiatives and quality assurance processes.
• Maintain accurate and up-to-date client records.
