Job summary

Placements24 is seeking an experienced Elderly Care Specialist in Benoni, Gauteng. The role focuses on assessing needs, creating personalized care plans, coordinating services, and advocating for seniors’ well-being within the community.

You will collaborate with families and healthcare professionals to deliver comprehensive care, monitor outcomes, and stay updated on geriatric best practices. A valid driver’s license and willingness to travel within Benoni are preferred.

Qualifications

• Relevant qualifications in Nursing, Social Work, Gerontology, or Occupational Therapy.
• Minimum of 4 years’ experience in elderly care, case management or related healthcare role.
• Strong knowledge of geriatric health issues and mobility challenges.
• Excellent assessment, planning and interpersonal skills.

Responsibilities

• Conduct comprehensive assessments of elderly clients’ needs.
• Develop and monitor individualized care plans with families.
• Coordinate medical appointments, home care and social activities.
• Provide emotional support and companionship to seniors.
• Educate families on caregiving resources and strategies.
• Liaise with doctors, nurses and therapists for integrated care.
• Advocate for the rights and needs of elderly clients.
• Maintain confidential client records and monitor care plan effectiveness.
