- Excellent interpersonal and remote communication skills.
Responsibilities
- Provide direct support to clients navigating rehabilitation programs.
- Assist clients in developing and achieving personal rehab goals.
- Offer emotional support and active listening.
- Help clients develop life skills and coping mechanisms.
- Connect clients with community resources and networks.
- Monitor progress and report changes to the rehab team.
- Maintain confidential progress notes and records.
- Educate clients and families on rehab processes.
