Job summary

Placements24 seeks a compassionate Disability Support Practitioner in Bloemfontein to empower individuals with disabilities to live independently. Qualifications

• Disability care or related qualification (e.g., FET Certificate in Community Support, NQF Level 4).
• Experience with individuals with diverse disabilities is desirable.
• Understanding of person-centred support and disability rights.
• Excellent interpersonal and communication skills.
• Patience, empathy, and desire to support others.
• Ability to work independently and in a team.
• Willingness for training and development.
• Knowledge of disability services legislation in SA.
• Ability to manage challenging behaviours.
• Flexibility to work shifts including weekends/public holidays in Bloemfontein.

Responsibilities

• Provide direct support to individuals with disabilities per their support plans.
• Assist with personal care including hygiene, dressing, and feeding.
• Support with household tasks, meals, and medication reminders.
• Facilitate participation in community activities and social events.
• Promote independence and self-advocacy for clients.
• Maintain accurate, confidential records of support and progress.
• Communicate with clients, families, and other professionals.
• Ensure a safe, health-compliant environment; follow safety guidelines.
• Report changes in client condition to supervisor promptly.
• Uphold dignity, rights and choices of individuals in Bloemfontein.
