Job summary

Placements24 is seeking a dedicated Social Worker to join the Community Health team in Klerksdorp, North West. The hybrid role involves on-site fieldwork and remote administrative tasks to support individuals and families with psychosocial needs.

You will conduct assessments, provide counseling, develop care plans, connect clients with services, and advocate within the health system, contributing to improved community health outcomes in the North West province.

Qualifications

• Registered Social Worker with SACSSP in good standing.
• BSW or equivalent required with post-qualification experience.
• Experience in counseling, case management and advocacy.

Responsibilities

• Conduct psychosocial assessments for individuals, families and groups.
• Provide counseling and support for health-related issues and life transitions.
• Develop and implement individualized care plans with healthcare teams.
• Facilitate access to community resources and services.
• Advocate for clients within the healthcare system.
• Deliver health education and promotion programs in the community.
• Collaborate with doctors, nurses and other professionals for integrated care.
• Maintain confidential client records and prepare reports.
