Placements24 is seeking a dedicated Social Work Case Manager to join their Durban team. This role blends remote and in-person interactions to support vulnerable individuals and families, assess needs, develop intervention plans, and connect clients with community resources to improve wellbeing.

You will maintain detailed case notes, monitor progress, provide counseling when needed, and collaborate with other professionals to ensure coordinated care in a hybrid work environment.

Qualifications

• Registered Social Worker with current professional body registration.
• Minimum of 3 years of experience in case management or social work.
• Strong knowledge of Durban's social welfare resources.
• Excellent communication and interpersonal skills.
• Ability to work independently in a hybrid environment.

Responsibilities

• Conduct comprehensive assessments of client needs, both remotely and through in-person visits.
• Develop, implement, and monitor individualized case management plans.
• Connect clients with appropriate social services, healthcare providers, and community support networks.
• Provide counseling and crisis intervention support as needed.
• Maintain accurate and detailed case notes and progress reports in the electronic system.
• Collaborate with other professionals and agencies to ensure coordinated care for clients.
