Placements24 is seeking a committed Case Manager to join our Durban team, delivering essential social services to individuals and families. This on-site role focuses on needs assessment, plan development, and guiding clients toward stability and self-sufficiency.

You will coordinate access to housing, employment, and healthcare resources, provide ongoing support, and maintain confidential records while collaborating with other service providers to ensure comprehensive client care.

Qualifications

• Diploma or Bachelor's degree in Social Work, Psychology, or related field.
• Minimum of 2 years of experience in case management or direct client support.
• Strong understanding of social service systems and Durban resources.
• Excellent empathy, active listening, and problem-solving skills.
• Ability to manage a caseload effectively and prioritize tasks.
• Proficiency in basic computer skills and case management software.

Responsibilities

• Conduct thorough needs assessments for clients seeking social services.
• Develop personalized case management plans outlining goals and intervention strategies.
• Connect clients with appropriate community resources, such as housing assistance, employment services, and healthcare.
• Provide ongoing support, monitoring, and follow-up to ensure clients are meeting their goals.
• Maintain detailed and confidential client records, documenting progress and interactions.
