About the Role

Our client, a well-established community services provider, is looking for a proactive and organised Community Care Coordinator to manage and enhance support services for residents in Port Elizabeth, Eastern Cape . This role is crucial in ensuring seamless delivery of care, facilitating communication between clients, families, and service providers, and coordinating various community-based programs. You will be instrumental in identifying client needs, developing care plans, and connecting individuals with the appropriate resources to improve their quality of life.
