Job summary

Placements24 is seeking a Social Care Facilitator to serve the George, Western Cape community. You will connect vulnerable individuals and families with essential support services and act as a vital link within the local social welfare network.

The role requires empathy, proactive problem-solving, and the ability to navigate complex community systems. A relevant degree and driver’s license are important for providing effective, on-the-ground support.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, or related discipline.
• Minimum of 2 years of experience in a social care or community support role, preferably within the Western Cape.
• Knowledge of local social services, NGOs, and government programs in the George area.
• Excellent interpersonal and empathetic communication skills.
• Proficiency in case management and record-keeping.
• Ability to work under pressure and manage multiple caseloads.
• Valid South African driver’s license and access to reliable transport.

Responsibilities

• Assess the needs of clients and provide appropriate social care interventions.
• Facilitate access to housing, healthcare, education, and employment services.
• Develop and maintain relationships with community partners and service providers in George.
• Offer counseling and emotional support to clients experiencing difficulties.
• Assist clients in developing life skills and achieving self-sufficiency.
• Document client interactions, progress, and outcomes meticulously.
• Represent the organization at community events and forums.
• Ensure compliance with policies and ethical standards.
• Collaborate with internal teams to share insights and improve service delivery.
• Respond effectively to client emergencies and escalating issues.
